Level of Service Questionnaire

This Level of Service Questionnaire has been designed to complement the Asset Management Plan for the Municipality of Trent Lakes and to link policy decisions about municipal infrastructure to long term financial costs. Your views and opinions will be used to determine if existing assets are performing up to expectation and where improvements may be required.

This form can be completed and submitted online for your convenience.

Questionnaires are also available at various locations in our Municipality including the Municipal Office, Public Libraries branches and all the Waste Transfer Stations. These completed forms may be mailed to the Municipality of Trent Lakes, 760 County Road 36, Trent Lakes, ON, K0M 1A0 or dropped off at the Municipal Office, Transfer Stations, or Libraries. The questionnaire is also available in alternate format. Please contact our office and let us know how we can assist you.
